Health & Social Care Integration – Consultation – Clackmannanshire & Stirling Partnership
Integration of health and social care is the Scottish Government?s programme of reform to improve services for people who use adult health and social care services. It will ensure that health and social care provision across Scotland is more joined-up and seamless, especially for people with long term conditions and disabilities, many of whom are older people.
Please find below the link to a new website which contains the draft vision and outcomes and draft integration scheme which are being consulted on until 31 January 2015. Also available on the website is a frequently asked questions section and who to contact for further information. Whilst there are two partnerships in the Forth Valley area (Falkirk Partnership and Clackmannanshire & Stirling Partnership) we are hosting only one website with information to reduce duplication.
